
The main risk factor for CSCC development is ultraviolet exposure, which induces DNA damage and leads to a high tumor mutational burden (TMB). 10, 11, 12 Indeed, the rationale for using a PD-1 checkpoint inhibitor in CSCC is strong. Programed cell death 1 (PD-1) immune checkpoint inhibition has been shown to be highly effective in patients with metastatic or advanced CSCC not amenable to curative surgery and/or radiotherapy with response rates ranging 34–50%. 6, 7 Despite aggressive local treatment, approximately 30% of patients with locoregionally advanced, resectable CSCC will recur and eventually die of disease.
4, 5 Standard treatment of resectable locoregionally advanced CSCC is surgery followed by adjuvant radiotherapy, which can cause significant disfigurement and functional morbidity.
